Nurse Practitioner/Physician Assistant – South Central Pennsylvania

• Federally Qualified Health Center (FQHC) seeks a Board-Certified Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join an established primary care team
• Salary range: 125-135k+ DOE
• Qualified for HRSA / National Health Service Corps (NHSC) Federal Student Loan Repayment due to FQHC status
• Outpatient schedule with standard daytime hours and one evening shift per week; no hospital rounding
• Shared call is light and strictly phone-based after hours, supported by a triage answering service
• Work within a Patient-Centered Medical Home (PCMH) model, paired with a dedicated MA, RN/LPN, and Integrated Behavioral Health Consultant
• Join a primary care medical staff of 8 to 12 providers, including Family Medicine Physicians, Pediatricians, and APPs, alongside dental and LCSW support
• Patient volume averages 16 to 22 patients per day across all age groups (pediatric to geriatric)
